What is Molina Healthcare?

Molina Healthcare is a multi-state health care organization that contracts with state governments to provide services through Medicaid and State Children’s Health Insurance Program (SCHIP). Molina also offers Medicare Advantage and Integrated Medicare-Medicaid plans (MMP).

Who is eligible for Molina Health coverage?

Individuals and families eligible for Medicaid and SCHIP can receive coverage from Molina Healthcare in states where benefits are offered including:

  • Children up to age 19
  • Pregnant members
  • Adults age 65 and older
  • Individuals with disabilities
  • Adult enrollees

Does Molina cover incontinence supplies? 

Molina Healthcare will only cover incontinence supplies from their preferred incontinence provider network. No prior authorization is necessary for these providers.

Molina’s Medicare supplemental benefits provides a monthly stipend for over-the-counter items, including incontinence supplies including protective underwear, briefs, pads, liners, cleaners, cloths, and creams.

How to get prior authorization for incontinence supplies through Molina Healthcare?

Incontinence supplies ordered outside of Molina’s preferred incontinence provider network will require a prior authorization. Patients who require more than six (6) incontinence garments per day will also need prior authorization.

Download the Prior Authorization Service Request form and have a healthcare provider submit for approval. 

What information is required for prior authorization?

Supporting information is required to grant prior authorization including:

  • Patient medical history (up to last 6 months)
  • Physical examination
  • Lab results (if applicable)
  • Speciality consultation notes
  • Other information specific to prior authorization request
